Fra i fattori di rischio emergenti un ruolo importante è giocato dalla genetica. Una storia familiare positiva per malattia cerebrovascolare è spesso considerata un fattore di rischio per ictus. Esistono tre tipi di studi di epidemiologia genetica volti a valutare l'associazione fra storia familiare e insorgenza di ictus: studi di popolazione, studi familiari e studi su gemelli.

La sindrome della apnee ostruttive del sonno (OSAS) è un disturbo presente in circa il 4% della popolazione generale di media età, raggiungendo valori molto più elevati nella popolazione sopra i 60 anni (>20%).[294] L'OSAS è caratterizzata dalla presenza di episodi ripetitivi e periodici di ostruzione delle alte vie aeree in corso di sonno, che si accompagnano a desaturazioni di ossigeno e a variazioni continue della pressione arteriosa e della frequenza cardiaca.

Una delle possibili localizzazioni della malattia aterosclerotica è rappresentata dall’arco dell’aorta. I fattori che si associano alla presenza di placche dell’arco aortico sono l’età avanzata, la presenza di ipertensione arteriosa, ipercolesterolemia e fumo di sigaretta ma anche fattori di rischio vascolare emergenti quali iperomocisteinemia, aumento dei globuli bianchi e dei valori della proteina C reattiva [Kronzon,2006].
